چکیده

Osteochondral lesions of the talus are a common cause of ankle pain. The optimum treatment for these injuries continues to evolve. The article describes an all-arthroscopic technique, which includes debridement, marrow stimulation, and application of a micronized allograft cartilage matrix with fibrin sealant. The procedure avoids potential disadvantages associated with other commonly performed procedures including necessity of arthrotomy or osteotomy, donor-site morbidity, increased cost, and need for multiple procedures. The technique is presented along with promising early clinical results. Level of Evidence: Diagnostic Level 4. See Instructions for Authors for a complete description of levels of evidence.

Arthroscopic Revision Repair of Osteochondral Lesion of Talus Using Microfracture Drill and Bioengineered Dehydrated, Micronized Allogenic Cartilage Extracellular Matrix

Osteochondral lesions (OCD) of the talus are a common cause of chronic ankle pain and dysfunction, especially with history of injury and trauma to the ankle [1-3]. Lesions that do not respond to non-operative treatment may require surgical intervention arthroscopically or as an open procedure. Revision repair of osteochondral lesions historically has been performed as an open procedure with inc...

A Review of Arthroscopic Bone Marrow Stimulation Techniques of the Talus

Osteochondral lesions of the talus are common injuries following acute and chronic ankle sprains. Numerous surgical treatment strategies have been employed for treating these lesions; arthroscopic bone marrow stimulation is recognized as the first-line technique to provide fibrocartilage infill of the defect site. While the short- and medium-term outcomes of this technique are good, the long-te...

Treatment of Osteochondral Lesions of the Talus With Bone Marrow Stimulation and Chitosan-Glycerol Phosphate/Blood Implants (BST-CarGel).

Bone marrow stimulation (BMS) techniques represent the first-line treatment for unstable osteochondral lesions of the talus or after conservative treatment failure. These techniques are intended to penetrate the subchondral bone to elicit bleeding and allow precursor cells and cytokines from bone marrow to populate the lesion. However, the fibrocartilaginous repair tissue arising after marrow s...

Allograft juvenile articular cartilage transplantation for treatment of talus osteochondral defects.

UNLABELLED Osteochondral (OCD) lesions of the talus are frequent sequelae of traumatic ankle injuries such as ankle sprains, fractures, and recurrent ankle instability. Initial management of talus lesions in most cases involves arthroscopy and microfracture/curettage. Tissue resulting from the microfracture is fibrocartilage. Clinical improvement in pain is seen in approximately 75% to 85% of p...

Use of Cartilage Extracellular Matrix and Bone Marrow Aspirate Concentrate in Treatment of Osteochondral Lesions of the Talus

Osteochondral lesions of the talus are generally the result of trauma and often result in persistent pain and disability. Various treatment options have evolved to treat these challenging problems, ranging from conservative management to marrow stimulation and cartilage or chondrocyte transplantation. Microfracture is the first line of treatment for many of these lesions. However, there is conc...
